There is a specific kind of anxiety that comes with knowing your chair time is just as high, but your year-end distributions are lower. If your production numbers are sliding year-over-year, you aren’t imagining it—and you aren’t alone.
Three Forces Creating a “Perfect Storm”
The specialist landscape has shifted, and three specific forces are creating a “perfect storm” for the traditional periodontist:
- The Clinical Creep: Every year, more General Practitioners (GPs) move from simple restorations to placing their own implants. What used to be your “bread and butter” referral is now staying in-house.
- The Demographic Gap: Your loyal referral base is aging. As your long-term colleagues retire, they are being replaced by younger clinicians who were trained in implants during dental school and have a “keep it in-house” mindset.
- The DSO Effect: Aggressive DSO acquisitions mean that even if a GP wants to refer to you, corporate mandates often force them to refer to an internal specialist or a high-volume center.
The Cost of Waiting
This isn’t just a dip; it’s an erosion of your retirement nest egg. If your practice sees a modest 7.5% annual decline, the impact on your eventual sale price is compounding and dramatic.
On a practice collecting $1M–$2M, a three-year slide at that rate can result in a loss of $160,000 to $320,000 in equity value. You are essentially paying a “waiting tax” to stay in practice.
How to Pivot
If you aren’t ready to exit, you must change your value proposition to the community. Shift your marketing to focus on:
Complexity & Complications: Position yourself as the “Master of the Failing Implant.” When a GP’s weekend-course case goes wrong, you are the only one who can save it.
High-End Tissue Management: Double down on advanced hard and soft tissue grafting—procedures that remain far outside the comfort zone of most GPs.
The Specialist Standard: Emphasize the 3+ years of residency versus the “weekend certificate.” In a world of systemic health risks (diabetes, smoking), your expertise is the patient’s best insurance policy.
